My friend played a Paladin who had fallen and turned to evil. One of our encounters was to infiltrate a bank and break into a vault. He was chosen to be a distraction so my scout and the party's rogue could sneak off to the vault.
He planned to cause a distraction by speaking with the bank teller, who was a little old lady. She had terrible hearing, and after growing tired of having to repeat himself, the anti-paladin punched her in the head. Well, more like through her head, causing it to fly from her shoulders.
Later on in the campaign we were coming to a crucial point in the plot. We found ourselves in an office, and in front of us was a desk and a chair, the back of which was facing us. A person began speaking from the chair and it was the voice of the man who had served as an antagonist thus far in the story. So the anti-paladin decided it was a good idea to kill this guy immediately. He drew his sword and charged at the chair, plunging the blade through the back and through the man sitting in it. It was revealed to us that he had just killed the king and we were soon going to be wanted for regicide. So we all dipped out and traveled to another country.
The DM actually had to change the entire campaign after that point because he hadn't anticipated losing a key NPC.
